TEAMWORK EVALUATION

Objectives:

§To give team members the opportunity to see how others perceive them as team members.

§To build trust among members.

§To open the lines of communication between team members.

Name:                                                             Date:

Directions:

§List alphabetically (by name), the members of your team. Include yourself in the list.

§Take one teamwork element at a time and consider each team member, including yourself. Determine who is “excellent” and who is “poor”. Wish the range in mind; rate the rest of the group. Use “Element of Teamwork” as a guide.

§Use the following rate scale: 1 = Poor 2 = Fair 3 = Average 4 = Good 5 = Excellent (Use “Average” as little as possible)

When you are rating, please consider each element and go across, determining which team member is “best” at communicating, for example. Do not consider each person individually and work down vertically.
